Real Server is failing health checks, found "Connection Failed - Server Unavailable" in the log files.

Question/Problem Description:

I can see some errors in the logs which correlate with the timings the issues occurred. However, I need some help deciphering the errors and root cause.

Steps to Reproduce:  
Error Message: Connection Failed - server unavailable
Defect Number:  
Enhancement Number:  
Cause: Health checks failed, but there was an active connection on the real server that failed health checks.
Resolution:

It should be determined why health checks are failing. This message will accompany another message in the logs, which provides a more concrete resolution for the reason health checks failed.
